Job summary

Hawthorne Center for Rehabilitation and Healing of Brandon is seeking skilled and compassionate Licensed Practical Nurses to join our team. You will provide direct care to residents in a nurturing, team‑oriented setting, delivering bedside nursing with empathy and professionalism.

This role includes shifts with new wage scale and $2 per hour overnights/weekends differentials. We offer comprehensive benefits, PTO, retirement options, and support for professional growth in Brandon, FL.

Qualifications

• LPN license valid in the state or multisate.
• CPR certification required.
• Two years nursing experience preferred.

Responsibilities

• Admissions, discharge and transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• Follow infection control, emergencies, and center policies; collaborate with interdisciplinary team.
• Identify and report abnormal changes in residents’ condition to proper personnel.
• Delivers nursing care to residents requiring long-term or rehabilitative care.
• Collects patient data, makes observations, and reports pertinent information.
• Implements and evaluates the patient care plan per state regulations.
• Direct and supervise care provided by other nursing staff when appropriate.
• Maintain knowledge of documentation requirements and equipment use.

Some of your Licensed Practical Nurse (LPN) Job Responsibilities include:

• Admissions, Discharge and Transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Following infection control, emergencies, and other center policiesCollaborating with resident’s interdisciplinary team to provide the best outcomes
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.
• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.
• According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.
• In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.
• Maintains knowledge of necessary documentation requirements.
• Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.
• Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.

Licensed Practical Nurse (LPN) QUALIFICATIONS:

• LPN (Licensed Practical Nurse) licensed by the State or Multistate License
• CPR Certification required
• Two Years prior nursing experience preferred
