Job summary

TimberRidge Center for Rehabilitation & Healing seeks a Licensed Practical Nurse (LPN) to support admissions, medication administration, and ongoing resident care. The role collaborates with an interdisciplinary team to ensure high-quality outcomes and adherence to care plans in a dynamic rehabilitation setting.

Applicants must be licensed LPN in the state, CPR certified, and ideally have at least two years of nursing experience.

Qualifications

• LPN licensed by the State or Multistate License.
• CPR Certification required.
• Two Years prior nursing experience preferred.

Responsibilities

• Admissions, Discharge and Transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• Following infection control, emergencies, and other center policies.
• Collaborating with resident’s interdisciplinary team to provide the best outcomes.
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy.
• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.
• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.
• Implements the patient/resident plan of care and evaluates the patient/resident response.

• Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.

Licensed Practical Nurse (LPN) QUALIFICATIONS:

• LPN (Licensed Practical Nurse) licensed by the State or Multistate License
