Job summary

Meadowbrook in Wisconsin is seeking a Licensed Practical Nurse (LPN) to provide direct resident care, administer medications, and collaborate with RNs and CNAs to meet residents’ physical and emotional needs. We offer both part-time and full-time AM/PM/NOC shifts, a $7,000 sign-on bonus, paid vacation, holidays and sick time, plus a 401(k) with employer match.

Qualifications

• Licensed Practical Nurse (LPN) in good standing.
• Experience delivering direct resident care and med administration preferred.
• Ability to collaborate with RNs, CNAs, and interdisciplinary team.

Responsibilities

• Administer medications and treatments per care plans.
• Provide direct resident care including vital signs and wound care.
• Document resident care in medical records.
• Collaborate with nursing staff on care plans and updates.
• Monitor resident conditions and report changes promptly.

Key Responsibilities

• Administer medications and treatments according to care plans and facility policies
• Provide direct resident care, including vital signs, wound care, and daily assessments
• Document resident care accurately in medical records
• Collaborate with RNs, CNAs, and interdisciplinary team members on care plans
• Monitor resident conditions and report changes promptly
• Support residents’ emotional well-being and comfort
• Follow infection control and safety procedures consistently
• Attend in-service trainings and professional development opportunities
• Complete additional duties as assigned by the Nursing Director or Administrator
