Job summary

Meadowbrook is seeking a Licensed Practical Nurse (LPN) to provide direct resident care, administer medications, and collaborate with the care team in Wisconsin. You’ll help meet physical and emotional needs while maintaining high standards of safety and professionalism.

We offer a supportive, team-focused environment with training and resources to help you grow. A $4,000 sign-on bonus is available, along with part-time, full-time, and casual shifts (AM/PM/Overnight).

Qualifications

Responsibilities

• Administer medications and treatments according to care plans and facility policies
• Provide direct resident care, including vital signs, wound care, and daily assessments
• Document resident care accurately in medical records
• Collaborate with RNs, CNAs, and interdisciplinary team members on care plans
• Monitor resident conditions and report changes promptly
• Support residents’ emotional well-being and comfort
• Follow infection control and safety procedures consistently
• Attend in-service trainings and professional development opportunities
• Complete additional duties as assigned by the Nursing Director or Administrator
