Meadowbrook offers an opportunity for Licensed Practical Nurse (LPN) to provide direct resident care in a supportive, team-focused setting. You will administer medications, monitor residents, and collaborate with RNs, CNAs, and the broader care team to implement care plans that address physical and emotional needs.

Responsibilities

• Administer medications and treatments according to care plans and facility policies
• Provide direct resident care, including vital signs, wound care, and daily assessments
• Document resident care accurately in medical records
• Collaborate with RNs, CNAs, and interdisciplinary team members on care plans
• Monitor resident conditions and report changes promptly
• Support residents’ emotional well-being and comfort
• Follow infection control and safety procedures consistently
• Attend in-service trainings and professional development opportunities
• Complete additional duties as assigned by the Nursing Director or Administrator
