Job summary

Sweetwater Care in Anaconda, MT is seeking an LPN with nursing management experience to provide nursing care within scope of practice and ensure patients' needs are met according to standards and physician orders.

You will participate in quality improvement, educate families and patients, and collaborate with other departments to ensure timely medication administration and treatments while adhering to facility policy.

Qualifications

• Licensed as LPN in the practicing state.
• Minimum one year nursing management experience preferred.
• Experience in long-term care setting required.
• Experience with PointClickCare charting preferred.

Responsibilities

• Demonstrates knowledge and appropriate use of Infection Control Manual.
• Participates in Quality Improvement Program implementation.
• Uses Quality Life Manual for patient placement and monitoring.
• Notifies departments of patient changes (room changes, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely according to facility policy.
• Administers treatments timely according to facility policy.
• Receives, transcribes, and carries out physician orders when allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications
