Job summary

Sweetwater Care in Montana is seeking an LPN with nursing management experience to lead clinical care within our facility. The role emphasizes patient advocacy, adherence to policy, and collaboration with physicians.

You will oversee medication administration, participate in quality improvement, and mentor staff while ensuring compliance with state standards and center procedures.

Qualifications

• Must be currently licensed as LPN in Montana.
• One year of nursing management experience preferred.
• Experience in long-term care setting is required.
• Proficiency with PointClickCare charting is a plus.

Responsibilities

• Provides nursing care within scope of practice per standards of practice and physician orders.
• Demonstrates knowledge and use of Infection Control Manual.
• Participates in Quality Improvement Program implementation.
• Notifies departments of patient changes and admission/discharge events.
• Educates patients and families about care and treatments.
• Administers medications and treatments timely per facility policy.
• Reads and carries out physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians when needed.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications
