Job summary

Human Touch Home Health is seeking a compassionate Licensed Practical Nurse (LPN) for PRN home health visits in Allentown and surrounding Lehigh Valley communities. You’ll provide one-on-one skilled nursing care in patients’ homes, with flexible scheduling that fits your life.

You'll administer medications, perform nursing assessments, educate patients and caregivers, and collaborate with an inter-disciplinary team under RN supervision. A PA LPN license and CPR/BLS are required.

Qualifications

• Active Pennsylvania Licensed Practical Nurse (LPN) license required.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Strong nursing, communication, and documentation skills.
• Ability to work independently in patients' homes.
• Home health experience preferred, but not required.
• WellSky experience is a plus.
• A genuine commitment to compassionate, patient-centered care.

Responsibilities

• Provide skilled nursing visits in patients' homes.
• Perform focused nursing assessments and report changes in patient condition.
• Administer medications and treatments as ordered.
• Provide wound care and monitor healing.
• Reinforce disease-management and medication education.
• Monitor vital signs and patient response to treatment.
• Educate patients and caregivers on safety and the established plan of care.
• Communicate changes and concerns promptly to the supervising RN and care team.
• Collaborate with nurses, therapists, physicians, and other members of the interdisciplinary team.
• Complete timely and accurate clinical documentation using WellSky.
