Castleview Hospital in Price, UT is seeking a Licensed Practical Nurse (LPN/LVN) to support nursing care under RN supervision. You will assess patients, participate in care planning, perform wound care, and assist with daily living activities while ensuring accurate documentation and communication with the care team.

The role requires an approved LPN/PN program graduate, UT-licensed or compact eligible, and BCLS certification within 30 days.

Qualifications

• Graduate of an approved practical nursing program.
• Current LPN/PN license to practice in UT or a UT-endorsed compact license.
• BCLS certification within 30 days of hire/transfer.
• Knowledge of ICD-9, ICD-10, CPT codes and insurance benefits.

Responsabilités

• Assist RN with patient assessment and data collection.
• Perform plan-of-care interventions and wound care; may administer medications.
• Assist RN with development of the plan of care.
• Assist patients with activities of daily living and hygiene.
• Monitor, document, and communicate patient status to RN/physician/family.
• Document patient care performed and advocate for patient well-being.
• Schedule appointments and procedures as ordered by the provider.
• Obtain insurance re-authorizations as needed.

Howyou'llcontributeas a LPN/LVN

Essential Functions

• Under the direction of an RN, contributes to the assessment of patients by collecting data for analysis by an RN.
• Performs plan of care interventions, including specimen collection and wound care. May administer medication.
• Assists RN with development of plan of care.
• Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
• Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.
• Documents patient care given.
• Works as an advocate for the physical and emotional well-being of the patient.
• Schedules appointments and procedures as ordered by the provider.
• Obtains insurance re-authorizations as requested for testing and scheduled procedures.

Non-Essential Functions

• Attends clinic meetings.
• Maintains a clean and safe working environment.
• Reports all incidents, hazards, drug interactions, and initiate appropriate action.
• Completes safety instructional programs.
• Assists other team members when time permits.

FLSA: Non-exempt

Education: Graduate of an approved practical nursing program

License: Current LPN/PN to practice in the Commonwealth of UT OR a compact License with UT endorsement. Also be in good standing with the Board of health Professions

Certifications: BCLS within 30 days of hire/transfer

Minimum Work Experience:

Knowledge of ICD-9, ICD-10, CPT codes, insurance benefits and healthcare backgrounds Required
