Job summary

RHA Health Services, LLC in Sparta, NJ is hiring a Licensed Practical Nurse (IDD LPN) for the Day Shift. This onsite, full-time role offers $34/hr with a $3,000 sign-on bonus and a Monday–Friday schedule, 8:30am–4:30pm.

The LPN will provide holistic nursing care to individuals with intellectual and developmental disabilities, perform data collection, and support medical care planning with the team. Office-based work supports group homes and routine visits.

Qualifications

  • Graduate of an accredited licensed nurse program.
  • Current LPN license with no restrictions to practice nursing.
  • Current CPR certification or eligible to certify for CPR.
  • Must meet credentials required by Federal or State law.
  • Ability to communicate effectively with the Interdisciplinary Team and families.
  • Ability to work independently, stay organized, and prioritize tasks.
  • Must be at least 18 years old.

Responsibilities

We are hiring for: Licensed Practical Nurse (IDD LPN) – Day Shift

Location: Sparta, NJ (onsite office based). Employment Type: Full-time. Compensation: $34/hr. Incentives: $3,000 sign-on bonus is being offered at this time. No relocation package offered. Schedule: Monday - Friday, 8:30am - 4:30pm.

This position is office-based and supports our group home operations with routine visits to residential homes. It is ideal for nurses seeking a more structured and steady-paced environment outside of traditional fast-paced hospital or SNF settings.

Job Responsibilities
  • Provide holistic nursing care to individuals with intellectual and developmental disabilities
  • Observe, assist with gathering health related data, and report the health needs of the people we support to the registered nurse
  • Coordinate preventative and routine medical care for people supported
  • Document nursing care provided
  • Assist physicians with on-site medical clinics where applicable
  • Provide health related training to unlicensed personnel according to state regulations
  • Conduct medication pass observations in the home to ensure staff competency according to state regulations
  • Conduct medication closest assessments
  • Perform quality assurance checks and observations to ensure unlicensed personnel are performing health related tasks as assigned according to state regulations
  • Observe for and report any abnormal health related findings to the physician
  • Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team
  • Advocate for the people supported and assist with developing community supports to promote optimal health for the people supported
Minimum Qualifications
  • Graduation from an accredited licensed nurse program
  • Current LPN license with no restrictions to practice nursing
  • Current CPR Certification or eligibility to certify for CPR
  • Possess credentials required by Federal or State law
  • Must be able to communicate effectively with Interdisciplinary Team Members, as well as the people supported and their families
  • Must be able to work independently, be organized, and possess the ability to prioritize work tasks
  • Must be at least 18 years of age
Physical Demands and Work Environment
  • Must be able to regularly lift and carry up to 20 lbs. and occasionally pull at least 40 lbs.
  • Must be able to squat, kneel, crawl, crouch, climb, and stoop as part of regular job duties
  • Must be able to use hands and fingers to handle or operate objects, tools, or controls
  • Required to stand and walk for extended periods
  • Must be able to demonstrate proficiency in CPR from floor level, which requires working on hands and knees, bending, standing, and lifting
  • Vision requirements include close, distance, and peripheral vision
  • Must be able to talk and hear. Exception – Deaf and Hard of Hearing Programs: For these programs only, employees must be fluent in American Sign Language (ASL). The requirement to talk and hear does not apply.
