Job summary

Sunrise Community, Inc. is seeking an LPN to provide essential healthcare services for individuals with intellectual and developmental disabilities in a hands-on setting. The role includes monitoring vital signs, assisting with daily health tasks, and accurate documentation.

Candidates must have a valid LPN license, a clean driving record, and basic computer skills. The position requires collaboration with Direct Support Professionals to support person-centered plans.

Qualifications

• Must successfully complete the required pre-employment Background and Drug Screenings.
• Must possess a valid Driver's License with all necessary endorsements and have a minimum of 3 years of driving experience.
• Must possess basic computer skills.
• Must possess current and valid license for Practical Nurse.
• Experience working with people with intellectual/developmental disabilities preferred.

Responsibilities

• Assists in providing basic healthcare services according to the individual’s plan ensuring each individual’s optimal health and wellbeing.
• Performs tasks as delegated, such as blood pressure monitoring, weights, and other assigned tasks.
• Ensures all documentation is accurate and complete according to regulations.
• Responsible for the organization and cleanliness of supplies and equipment, ensuring restocking of inventory.
• Collaborates with Direct Support Professionals to ensure that the needs of the individuals are being met.
• Communicates immediately any changes in medical or behavioral status to supervisor.
• Performs all other duties as assigned.
