Job summary

Halifax Health ExpressCare in Florida is seeking a Day/Night Hospice Field Licensed Practical Nurse (LPN) to provide direct patient care to hospice patients across diverse settings under the supervision of an RN. Shift coverage includes days, nights, weekends and holidays to ensure 24/7 care.

Day (United States of America) Licensed Practical Nurse- Hospice (Field) The Field Licensed Practical Nurse provides direct patient care to hospice patients in a variety of settings and levels of care under the direct supervision of an RN. Days/Nights/Weekends/Holidays as assigned to provide 24 hours a day, 7 days a week care assigned to a variety of teams/duties.

Responsibilities include:

• Providing care to the terminally ill patient
• Reinforcing patient and family training and education
• Performing patient assessment and documentation
• Participating in the comprehensive Interdisciplinary Plan of Care
• Participating in the Quality Improvement activities

All services will be provided 24 hours a day, 7 days a week as assigned to a variety of teams and duties. Nights and weekends. On-Call rotations as assigned.

JOB QUALIFICATIONS:

• Valid Driver's license with a good driving record
• Personal car in good working order
• Valid Florida license for Licensed Practical Nurse (LPN) BLS and IV certification per department requirement
• Minimum 1 year experience in Medical Surgery Nursing or equivalent Hospice experience preferred
• Graduated from an accredited nursing program, must meet criteria for employment by Halifax Health
