Job summary

Pathways Healthcare is seeking a compassionate Licensed Practical Nurse (LPN) to join our growing team for direct home health patient care. You will collaborate with RNs, physicians, patients, and families to ensure high-quality care and positive outcomes.

Responsibilities include assessments, medication administration, care plan implementation, patient education, and accurate documentation. Per visit rate; flexible schedule and mileage reimbursements are offered.

Qualifications

• Current and active LPN license in Massachusetts.
• Graduate of an accredited practical nursing program.
• Previous nursing experience preferred, especially in home health, hospice, or geriatrics.
• Proficient with electronic medical records (EMR).
• Excellent communication and interpersonal skills.
• Current BLS CPR certification.
• Valid driver's license and reliable insured vehicle required.

Responsibilities

• Provide skilled nursing care under the supervision of a Registered Nurse and in accordance with the plan of care.
• Monitor and document patient condition, vital signs, symptoms, and responses to treatment.
• Administer medications and treatments as prescribed.
• Assist with implementation and ongoing evaluation of patient care plans.
• Educate patients and caregivers on medications, treatments, and disease management.
• Communicate changes in patient condition to the supervising RN and physician as appropriate.
• Maintain accurate and timely clinical documentation.
• Promote a safe and comfortable environment for patients.
• Coordinate necessary supplies, equipment, and services for patient care.
• Support quality improvement initiatives and compliance standards.
• Accept and fulfill assigned patient visits and schedules reliably.
