Job summary

Care 360 Hospice is seeking a Hospice Licensed Practical Nurse to provide skilled nursing care under RN supervision in Warren County, Ohio. The role covers Mason and Springboro and surrounding areas, with a 8:00 AM-5:00 PM schedule, Monday through Friday, full-time.

Salary ranges from $31.25 to $34.13 per hour based on experience. The LPN will support the plan of care, assist RNs with procedures, educate patients and families, and participate in ongoing professional development within a

Qualifications

• Must have a Licensed Practical Nursing degree.
• Minimum of 1 year of clinical experience.
• BLS certification required.
• Hospice experience preferred.
• Valid driver's license and auto insurance coverage.
• Reliable transportation required.

Responsibilities

• Provide skilled nursing care under the supervision of an RN.
• Assist Registered nurses in carrying out the plan of care.
• Assist RN in performing specialized procedures under physician's orders.
• Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current.
• Assist reinforcement of patient and family education regarding end-of-life care.
• Assume responsibility for personal and vocational growth through participation in nursing organizations and in-service learning.
• Keep the RNCM informed of changes noted during the process.
• Participate in self-development through education, workshops, and conferences.

About the Hospice LPN role:

• Provide skilled nursing care under the supervision of an RN

• Assist RN in performing specialized procedures( as defined in the state nurse practice act) as directed by the RN under physician's orders.
• Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed.
• Assist reinforcement of patient and family education regarding end-of-life care.
• Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.
• Keep the RNCM informed of changes noted during the process.
• Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research reading.
