Sentara Hospice in Chesapeake, VA is seeking a Licensed Practical Nurse (LPN) to join our home health care team. You will provide direct, one-on-one patient care, monitor health, administer treatments and medications, and collaborate with RNs and physicians to deliver high-quality, compassionate care.

The role is full-time on the night shift (12 AM–8:30 AM). Candidates must hold a VA LPN license and complete BLS within 90 days. A strong preference for hospice experience is noted.

Overview:

The Licensed Practical Nurse (LPN) in a home health care setting is responsible for providing direct, one-on-one care to patients in their homes. This role involves monitoring patient health, administering treatments, managing medications, and offering essential support to patients with various medical needs. Collaboration with a team of healthcare professionals, including Registered Nurses (RNs) and physicians, is required to deliver high-quality, compassionate care that improves patient well-being.

Anticipated Status/Shift:

Full Time (40 hours/week) Night Shift (12AM-8:30AM)

Position Location:

Chesapeake, VA

Hourly Range: $22.39-$37/hour

$3,000 Sign-On Bonus available for eligible candidates!

Education:

• Trade School Graduate for Nursing (Required)

Certification/Licensure:
