Job summary

Trilogy Home Health, a Humana Company, is seeking a compassionate LPN for home health visits in Naples, FL. You will provide direct patient care in homes or facilities, coordinating with physicians and the clinical team to optimize outcomes.

The role requires FL LPN licensure, graduation from an accredited nursing program, and IV certification, with 1 year of home health experience preferred. Full-time position with mileage reimbursement and strong benefits.

Qualifications
