Trilogy Home Healthcare, a Humana company, seeks a Licensed Practical Nurse (LPN) for Per Diem/PRN home health in Manatee County, FL. Provide compassionate, patient-centered care according to each plan of care and collaborate with physicians and RNs to support optimal outcomes.

Requirements: graduation from an accredited nursing program, active Florida LPN license, a valid Florida driver’s license with insurance, and at least 1 year of nursing experience (home health a plus).

Qualifications

• Graduate of an accredited school of nursing.
• Active LPN license in Florida.
• Minimum 1 year nursing experience; home health considered a plus.
• IV certification is preferred.
• Valid Florida driver's license with insurance and clean MVR.
