Job summary

Trilogy Home Healthcare, a Humana company, is hiring a dedicated Licensed Practical Nurse (LPN) to support home health patients in Polk County, FL. This PRN/Per Diem role offers flexible scheduling within a trusted, award-winning organization.

Ideal candidates hold an active Florida LPN license, have at least 1 year of nursing experience (home health a plus), and demonstrate strong initiative and independent judgement.

Qualifications

• Graduate of an accredited school of nursing.
• Active LPN license in Florida.
• Valid Florida driver’s license, active vehicle insurance, and clear MVR.
• Minimum 1 year nursing experience; Home Health experience a plus.
• IV certification preferred.
• Strong initiative and independent judgement.
