365 Health Services is seeking LPNs with availability for days, evenings, or overnight shifts in Northeast Philadelphia and Lower Bucks County.

Responsibilities include:

• Collecting comprehensive patient data, including physical, psycho-social, emotional, family, support system, and environmental status, to determine appropriateness for home care.
• Following written care plans developed by registered nurses, based on medical and nursing diagnoses, patient needs, and standards of care.
• Identifying patient care needs during shifts, reporting to the Clinical Supervisor, and coordinating care with the healthcare team, including agency staff, other agencies, and vendors.
• Ensuring compliance with home health documentation, communication standards, and infection control policies.
• Performing skilled nursing procedures as ordered, providing patient and caregiver education, and documenting all care activities accurately and timely.
• Monitoring vital signs, updating patient care plans, managing supplies, and demonstrating safe use of equipment and driving practices.

Job Details:
