We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. This part-time and PRN position offers flexible scheduling options to accommodate your availability. The ideal candidate will provide high-quality nursing care to patients, assist with their daily living activities, and work collaboratively within a multidisciplinary team to ensure optimal patient outcomes.

Key Responsibilities

• Provide direct patient care under the supervision of a Registered Nurse or physician, including administering medication, performing treatments, and monitoring vital signs.
• Assist in the development and implementation of individualized care plans based on patient assessments.
• Educate patients and their families on health management, medication administration, and self-care techniques.
• Collaborate with healthcare team members to provide comprehensive care and promote patient wellness.
• Maintain accurate and up-to-date patient records, documenting care provided and any changes in patient condition.
• Execute physician orders and follow facility protocols in adherence to state regulations.
• Assist with admissions, discharges, and transfers of patients as necessary.
• Participate in ongoing training and professional development to stay current with nursing practices and regulatory requirements.

Qualifications
