As a Home Health LPN, you will:

1. Provide professional nursing services under the supervision of an RN, complying with the state's Nurse Practice Act, licensure requirements, healthcare standards, laws, regulations, and CenterWell policies.
2. Deliver skilled nursing interventions for patient illness, rehabilitation, and preventative care, using a holistic approach aligned with the care plan.
3. Apply clinical knowledge and skills to facilitate problem resolution and achieve patient-centered goals and outcomes.
4. Consult with the RN Supervisor regarding treatment plan changes and communicate with physicians as permitted by law.
5. Implement the nursing plan of care with a systematic, goal-oriented approach.
6. Maintain communication with patients, families, physicians, and the healthcare team regarding patient needs and status, participating in care coordination and discharge planning.
7. Uphold confidentiality and professional conduct, sharing information only when appropriate and in the patient's best interest.
8. Ensure availability of supplies and equipment needed for safe and effective care.
9. Complete and submit documentation as required, including visit notes.

Required Experience/Skills:

• Graduation from an accredited LPN/Vocational Nursing program.
• Valid nursing license in the practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• At least two years of clinical experience as an LPN/LVN, preferably in home health or hospice.

Pay Range

The estimated starting pay for full-time (40 hours/week) employment is between $28.00 and $40.00 per visit/unit or $44,600 and $61,400 annually, depending on location and experience.
