Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Home Health team in Clarendon County. This full-time position offers a sign-on bonus and the opportunity to work in a dynamic healthcare environment.

Essential Functions

• Maintain a professional image and exhibit excellent customer relations with patients, visitors, physicians, and co-workers, adhering to our Service Excellence Standards and Core Values.
• Assist with patient revisits within a designated geographical area under the supervision of the Team Clinical Manager and the primary RN. Collaborate with interdisciplinary team members for comprehensive patient care.
• Demonstrate effective communication skills, attention to guest relations, good interpersonal relationships, and organizational skills. Computer literacy and good writing skills are desirable.
• Handle safety-related issues for staff and patients, assessing data reflective of patient status, and providing age-specific care. Exhibit maturity, self-motivation, ethical behavior, and independent judgment.
• Communicate efficiently with a diverse population, work closely with staff, physicians, and external agencies, and adhere to dress code and attendance policies. Maintain punctuality and a professional appearance.

Schedule

Full Time, 80 hours biweekly, based in Manning, SC (Clarendon County).

Qualifications/Training

• Minimum of one year of nursing experience; home care experience highly preferred.
• Previous home care experience is a plus.

Licenses/Certifications/Education

• Valid South Carolina or multi-state (compact) LPN license.
• Current valid SC driver's license and automobile insurance.
• BLS Certification.
• Must be at least 21 years old to meet McLeod Health fleet vehicle insurance requirements.
