Job summary

Pathways Healthcare is seeking a Licensed Practical Nurse - Home Health to join our patient-centered team serving Hyde Park, Mattapan, Roslindale, West Roxbury and surrounding areas. The main office is Westwood, MA, with a Monday–Friday schedule from 8:30 AM to 5:00 PM.

Ideal candidates have 2+ years of nursing experience (geriatrics preferred), a nursing degree, EMR proficiency, valid driver's license and reliable vehicle.

Qualifications

• 2+ years of nursing experience, geriatrics preferred.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records (EMR).
• Excellent bedside manner and communication skills.
• Knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Valid driver's license and reliable vehicle.

Responsibilities

• Assess and evaluate patient status and write/initiate plan of care.
• Regularly re-evaluate patient and family needs and revise care plan as needed.
• Develop care plans based on nursing diagnosis and patient/family input.
• Administer medications and treatments as prescribed.
• Document clinical notes and track pain/symptom changes.
• Communicate with physicians about patient needs and orders.
• Maintain a safe environment and participate in group meetings.
• Participate in on-call duties and coordinate equipment/services.
• Oversee ancillary staff and pursue ongoing professional development.

Job Description:

• Role: Licensed Practical Nurse - Home Health
• Main Office location: Westwood, MA
• Service Area: Hyde Park, Mattapan, Roslindale, West Roxbury and surrounding areas

• Job Type: Full Time

Responsibilities:

• Assesses and evaluates patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.
