Job summary

Amedisys is seeking a licensed practical nurse to deliver patient care in home settings on a part-time daytime schedule. You will verify plans of care with RNs, collaborate with physicians, educate patients and families, and document visits with compliance to policies.

The role requires an active LPN license, CPR certification, and a valid driver's license with reliable transportation. Benefit eligibility varies by position and shift status.

Qualifications

• Minimum one year of clinical LPN experience.
• Current LPN license valid for the assigned state(s).
• Current CPR certification is required.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs and report changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs at visits.
• Review medications and perform medication reconciliation.
• Perform procedures ordered by physician (e.g., Foley catheter changes).
• Prepare equipment and materials using aseptic technique.
• Complete timely and accurate documentation.
