Job summary

Amedisys is seeking a licensed practical nurse (LPN) for part-time days to support in-home patient care. As a key member of the care team, you will verify plans of care with the RN, coordinate with physicians, educate patients and caregivers, and perform vital sign checks.

If you have a current LPN license in the assigned state, CPR certification, at least one year of clinical LPN experience, a valid driver's license and reliable transportation, you may join a company offering competitive pay,

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; Notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. Foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.
