Base pay range

$44,600.00/yr - $61,400.00/yr

Role Responsibilities:

• Provide professional nursing services under the supervision of a RN, complying with all relevant laws, standards, and policies.
• Deliver skilled nursing interventions for treatment, rehabilitation, and prevention, using a holistic approach.
• Apply clinical knowledge to meet patient goals and resolve problems.
• Consult with RN Supervisor about treatment plans and communicate orders from physicians.
• Implement the nursing care plan with a goal-oriented approach.
• Maintain communication with patients, families, and healthcare team members regarding patient needs and status.
• Ensure confidentiality and proper handling of sensitive information.
• Manage supplies and documentation as required.

Minimum Qualifications:

• Graduate of an accredited LPN program.
• Valid nursing license in the practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• At least two years of clinical experience as an LPN/LVN, preferably in home health or hospice.
