Abcor Home Health is currently looking to hire Licensed Practical Nurse to service our homebound patients in the location listed above and surrounding areas.
Our main office is in Arlington Heights at 3201 N. Wilke Road, Arlington Heights, IL 60004.
Please note: LPNs working in Chicago and the south suburbs will report to our Midway office location at 6239 S. Kedzie Avenue, Chicago, IL 60629.

General Description:
A Licensed Practical Nurse (LPN) is a healthcare professional who provides patient care under the direction and supervision of Registered Nurses (RNs) and Physicians. The Licensed Practical Nurse administers skilled nursing care to patients intermittently in their place of residence. Their responsibilities include monitoring patients’ health, administering medications, dressing wounds, collecting samples for testing, and providing basic patient care. LPNs play a crucial role in ensuring patients receive quality care and support. Services are furnished in accordance with Agency policies.
Main Responsibilities of the role include but are not limited to:
Summary of Job Functions:
Understand and adhere to established policies and procedures.
Implement the nursing care plan for each patient.
Provide nursing services, treatments and diagnostic and preventive procedures as assigned.
Initiate preventive and rehabilitative nursing procedures as appropriate.
Observe signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
Evaluate with registered nurse the effectiveness of the LPN's/LVN's nursing service to the patient and family under the guidance of the registered nurse.
Maintain accurate and complete records of observations, treatments and care of patient.
Participate in medical record audit as assigned.
Attending staff meetings, patient care conferences and in-services as scheduled.
Take on-call duty, weekends and holidays as needed.
Prepare clinical and progress notes.
Assist the physician and RN in performing specialized procedures.
Prepare equipment and materials for treatments.
Assist the patient in learning appropriate self-care techniques
Position Qualifications:
LPNs must pass the National Council Licensure Examination for Practical Nursing (NCLEX-PN) with a license in their state.
Graduate of a state approved school of Practical (vocational) Nursing and currently licensed in the state in which they are practicing.
Minimum of one (1) year of experience in nursing, preferably.

Critical Thinking: able to assess patients’ conditions, changes in health status and respond appropriately.
Lifting Requirements:
Ability to participate in physical activity.
Ability to work for a long time while standing and involved in physical activity.
Heavy lifting.
Ability to do extensive bending, lifting and standing on a regular basis.
