Responsibilities

The Licensed Practical Nurse (LPN) in Home Health provides quality patient care and performs technical skilled care in the patient's home.

• Provides services within the scope of practice defined by state laws, following the plan of care, and coordinating with the healthcare team. May not alter the plan of care.
• Completes and submits progress notes daily for each visit.
• Observes and reports the patient's nursing needs; instructs family members in patient care and maintaining a healthy environment.
• Records, reports, and communicates the patient's response to treatment and any changes in condition to the supervising nurse or physician.

Education and Experience

License Requirements

• Current LPN licensure in the state of practice
• Valid driver's license, vehicle insurance, and access to reliable transportation
• CPR certification
