Avamere Transitional Care at Sunnyside in Salem, OR is seeking a Licensed Practical Nurse to provide basic nursing care under RN supervision. You will monitor residents' conditions, administer medications, perform wound care, and assist with daily activities, mobility, nutrition, and hydration.

This role emphasizes teamwork with CNAs and healthcare professionals to ensure safety and accurate documentation. A valid LPN license, CPR/BLS certification, and EMR documentation experience are

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, unencumbered, active license to practice as an LPN in this state.
• Must maintain and have an active CPR/BLS during employment.
• Prefer experience in long term care, geriatric rehab, sub-acute and or critical care clinical setting.
• Experience in electronic medical records documentation.

Responsibilities

• Provide basic nursing care including vital signs, medication administration, wound care, hygiene and mobility support.
• Record patient history and observe changes in condition, reporting to the RN.
• Work under RN supervision and supervise CNAs to provide care.
• Maintain patient safety and accurate documentation of care delivered.
