Licensed Practical Nurse (LPN) – Full-time 8‑hour shifts

Shift: 7:00 AM‑3:00 PM, 3:00 PM‑11:00 PM, and 11:00 PM‑7:00 AM.

LPNs in the Memory Care unit will receive a $1 shift differential for both 2nd and 3rd shifts.

Job Summary

Licensed Practical Nurse (LPN) will implement and coordinate the delivery of care in collaboration with physicians and care associates. Above all, the LPN will exhibit a passion for serving seniors and contributing to the positive environment residents are proud to call home.

• Assessment, planning, implementation, and evaluation of residents
• Provide high level of care by making frequent rounds, ensuring continuity of care, documenting all pertinent care information, and observing residents’ overall condition and behavior
• Assist the Director of Wellness in the development and update of resident care plans
• Maintain ongoing communication with residents, families, and physicians regarding the resident’s care
