Job summary

Bridges Senior Living is seeking a Licensed Practical Nurse (LPN) to coordinate care in our Memory Care unit, working with physicians and care teams to assess, plan, implement and evaluate resident care. You’ll provide compassionate, high‑quality nursing support and communicate clearly with residents, families and staff.

Shifts are 7:00–15:00, 15:00–23:00 and 23:00–07:00 with a $1 per hour differential for 2nd and 3rd shifts.

Qualifications

• High school diploma or GED.
• LPN license in applicable state.
• First Aid & CPR certification.
• At least 1 year of related work experience.

Responsibilities

• Assess, plan, implement, and evaluate resident care.
• Provide high level of care with frequent rounds and clear documentation.
• Assist the Director of Wellness in updating resident care plans.
• Maintain ongoing communication with residents, families, and physicians.

Licensed Practical Nurse (LPN) Job Summary:

• Assessment, planning, implementation, and evaluation of our residents
• Provide high level of care by making frequent rounds, ensuring continuity of care, documenting all pertinent care information, and observing our residents’ overall condition and behavior.
