The Columbia Presbyterian Community in Lexington, SC is seeking a Licensed Practical Nurse (LPN) to provide compassionate, individualized care for residents. You will conduct rounds, monitor conditions, and supervise other care staff, ensuring care plans are followed and documentation is accurate.

Qualifications

• Must hold valid LPN license in South Carolina.
• 1–2 years of healthcare experience preferred.
• Current CPR certification.

Responsibilities

• Provide compassionate, individualized care according to residents' care plans.
• Perform regular rounds and monitor residents' conditions throughout the shift.
• Supervise and direct other care staff.
• Communicate effectively with residents and families about care updates.
• Administer medications and treatments per physician orders.
• Complete documentation accurately and in compliance with regulations.
• Notify physicians and families of significant resident condition changes.
• Assist with resident admissions, discharges, and transfers.
• Maintain clinical competency and uphold professional standards.
• Able to work weekends, holidays, and according to resident needs.
