Job summary

Havenovacare is seeking compassionate Licensed Practical Nurses (LPNs) to provide in-home care across Massachusetts, with options for Full-Time, Part-Time, and PRN assignments. You’ll deliver hands-on nursing care in patients’ homes under RN or physician supervision, balancing a flexible schedule with high-quality patient outcomes.

Ideal candidates hold an active MA LPN license in good standing, have at least 1 year of clinical experience (home care preferred), CPR/BLS, a valid driver's license,

Qualifications

• Current LPN license in Massachusetts in good standing.
• Minimum 1 year of clinical nursing experience; home care preferred.
• Current CPR/BLS certification.
• Valid driver’s license and reliable vehicle.
• Ability to pass a criminal background check and health screening; weekend availability required.
• Bilingual (English + any additional language) strongly preferred.
• Proficiency with EHR systems is a plus.

Responsibilities

• Provide direct, in-home patient care including medications, wound and catheter care, and prescribed treatments.
• Monitor and document vital signs and health status; report changes to care team.
• Implement individualized care plans with RNs/physicians.
• Educate patients and families on meds, procedures, and disease management.
• Maintain accurate clinical documentation compliant with standards.
• Communicate progress with supervisors and care teams.
• Follow infection control, safety, and HIPAA regulations.
• Attend care conferences and team meetings as needed.

Are you a compassionate LPN looking to make a real difference in patients' lives — right in the comfort of their own homes? Havenovacare is hiring Licensed Practical Nurses for Full-Time, Part-Time, and Per Diem/PRN positions across Massachusetts. You will provide hands‑on nursing care to our patients under the supervision of a Registered Nurse or physician, with the flexibility to balance your schedule while delivering exceptional care. What You'll Do

• Provide direct, in‑home patient care including medication administration, wound care, catheter care, and prescribed treatments
• Monitor and document patient vital signs, health status, and any changes in condition
• Implement individualized care plans in collaboration with RNs, physicians, and the broader care team
• Educate patients and family members on medications, procedures, and disease management
• Maintain accurate, timely clinical documentation in compliance with agency and regulatory standards
• Communicate proactively with supervisors and care teams regarding patient progress
• Follow all infection control protocols, safety standards, and HIPAA regulations
• Attend care conferences and team meetings as needed

What We're Looking For

• Current, valid LPN license in the State of Massachusetts (in good standing)
• Minimum 1 year of clinical nursing experience; home care experience preferred
• Current CPR/BLS certification
• Valid driver's license and reliable personal vehicle
• Ability to pass a criminal background check and pre‑employment health screeningAvailability to work weekends (required)
• Bilingual (English + any additional language) strongly preferred
• Proficiency with electronic health record (EHR) systems is a plus
