Job summary

OPWDD seeks a licensed practical nurse to provide hands-on nursing care under the supervision of a registered nurse or physician. You will assist in developing care plans, administer medications, and monitor patient vital signs within our facilities.

The role emphasizes patient education, observation, and collaboration with the care team to support health goals and rehabilitation activities. A NYS license and valid driver's license are required.

Qualifications

• Hold a NYS license/registration to practice as a practical nurse.
• Possess a limited permit to practice as a practical nurse in NYS.
• If applying with a limited permit or after graduation, license must be obtained within one year.

Responsibilities

• Under the direction of a licensed health care provider, perform nursing care tasks per treatment plans.
• Administer medications and nursing procedures as prescribed.
• Record vital signs and monitor health status with standard instruments.
• Observe and report patient status and response to treatments to supervisors.
• Prepare patients for examinations and treatments; assist with procedures.
• Maintain patient charts and records of treatments.
