Duties include administering medications and treatments safely per the plan of care. Candidates must hold an active LPN license and demonstrate strong communication, time management, and critical thinking skills.

Qualifications

• Must hold an active Licensed Practical Nurse license.
• Experience in long-term care/rehab preferred.
• Excellent communication and organizational skills required.

Responsibilities

• Provide basic nursing care and ongoing resident assessment.
• Document assessments accurately and communicate changes to the IDT.
• Assist with care planning, nursing orders, and physician orders.
