Job summary

Alomere Health is seeking a part-time LPN/CMA to support outpatient clinic operations in Alexandria, MN. The role involves nursing tasks, patient care coordination, documentation in the EMR, and collaboration with providers. Flexibility to cover varying shifts and weekend rotation is required.

Qualifications

Responsibilities

• Meet and follow Meaningful Use, PQRS, and quality initiatives
• Maintain knowledge of Provider practice type

• Align with primary Providers' clinic schedule
• Obtain prior authorization of medications
• Prepare and administer injections and medications
• Administer treatments as prescribed

• Process charts on admission/discharge
• Handle specimens appropriately
• Facilitate communication between patient and Provider
• Document all patient interactions in EMR
• Improve patient experience and flow
• Communicate with patients and families
• Assist in emergent situations with crash cart/defibrillator
• Respond to patient and visitor requests
• Procedures in specialty areas as needed
• Perform other duties professionally
• Use relevant medical equipment

Employment Status: Part-Time (0.8) - 64 Hours Per Pay Period

Hours:

• Monday, Wednesday, Thursday, Friday: 7:45am to 5:00pm -

• Tuesday off

• As a float may occasionally be scheduled between the hours of 7:05am to 8:00pm
• Rotation of every 4th weekend

Position Objective:

The LPN/CMA supports the Provider in provision of individualized patient care, direct and indirect in an outpatient clinical setting. LPN/CMAs participate in the planning, implementation, and evaluation of nursing care, based on objectives, standards, and policies of nursing. They have the ability to handle a diverse range of situations and responsibilities, which is their primary duty. LPN/CMAs are responsible for rendering nursing care in an assigned area in terms of individualized patient needs, according to conformance with recognized nursing techniques, procedures, and established standards based on the scope of practice, under the direction/delegation of a Registered Nurse or Provider.

Essential Responsibilities:

Nursing Service Delivery:

• Meet, maintain, and follow Meaningful Use, PQRS, MN community Measures requirements, and other quality initiatives
• Adequate knowledge of Provider practice type
• Ensure efficient and accurate patient care coordination
• Perform all tasks per Provider order within scope of practice or specialty training
• Work in accordance of primary Providers' clinic schedule
• Possess prior authorization of medications

Alexandria Clinic Specific:

• Prepare and administer injections, immunizations, and medications
• Administer treatments as prescribed by Provider
• Submit appropriate charges for services rendered
• Process patients' charts upon admission and discharge
• Collect and handle all specimens appropriately

Patient Care:

• Facilitate communication in regards to patient care between patient and Provider and work to be a patient advocate at all times

• Complete thorough documentation of all patient interactions in the electronic medical record
• Improve the patient experience and improve patient flow within Alomere Health
• Effectively communicate with patients, families, and those involved in the patients' care
• Assist in emergent situations, using knowledge and skill of crash cart and defibrillator resources
• Respond to patient and visitor requests, complaints, and recommendations

Specialty Practice:

• Employees working in specialty areas will be required to perform procedures specific to that specialty

Other Responsibilities:
