Licensed Practical Nurse Care Manager / Case Management (LPN)

Brooklyn Center is hiring Licensed Practical Nurses (LPN) in Brooklyn, NY.

Compensation:
- Base rate: $31.94 with an additional 10% shift differential for evenings and nights.
- Non-union rate: $40/hour.

Responsibilities:

• Collect information from new residents for admission.
• Record health details, including vitals and temperature.
• Administer medications and injections as needed.
• Treat and dress wounds and bedsores.
• Supervise Certified Nursing Assistants (CNAs) if required.
• Assist residents with dressing and personal hygiene.
• Monitor food and liquid intake and output.

Requirements:

• Ability to work as part of a team.
• Valid LPN license in the state.
• Good standing with the State Registry.
