Job summary

Personal Healthcare Management LLC seeks two full-time evening LPNs and one full-time overnight LPN in New York. The roles provide safe, compassionate resident care and require accurate documentation and coordination with the nursing team.

Starting pay is $30/hr, up to $31/hr base, with evening differential to $32/hr and night differential to $32.25/hr. A comprehensive benefits package, 401(k), and paid time off are offered.

Qualifications

• 1 year of nursing experience
• Active LPN license
• Ability to work evenings and nights
• Medication administration and documentation competency

Responsibilities

• Provide safe resident care and administer treatments per policy
• Administer medications following the five rights and document faithfully
• Communicate care plans during handoffs and shift reports
• Participate in quality improvement and ensure age-appropriate care
• Collaborate with nursing management on problems and solutions
• Adhere to infection control and safety standards

Functions

• Provides safe resident care, demonstrating knowledge and competency in completing assigned treatments and procedures according to established standards and policies. Maintains awareness of new updates standards and incorporates them into his/her nursing practice throughout the continuum of care.
• Participates in the improvement process of quality resident care.
• Incorporates age-specific needs, provision of care, procedures, and treatments.
• Communicates pertinent resident information and plan of care before breaks, during transfer, and at shift report.
• Accurately administers medications according to the five rights of medication administration and documentation of same.
• Accurately performs treatments utilizing sterile or clean technique.
• Accurately administers external feedings.
• Completes resident care-related activities within scheduled shift time periods.
• Cooperates with reassignments made in the interest of resident care.
• Reports resident care problems and possible solutions to the appropriate nursing management personnel.
• Delegates resident care problems and possible solutions to the appropriate nursing management personnel.
