Job summary

Divine Rehabilitation and Nursing at St Croix in Wisconsin seeks a Licensed Practical Nurse to deliver direct patient care in collaboration with Registered Nurses and within the Nurse Practice Act, ensuring safe, compassionate treatment.

You will document care in residents' records, follow the therapeutic plan, assist with medications, reinforce physician orders, and report changes to the RN, while staying prepared to initiate CPR if needed.

Qualifications

• High school diploma or equivalent.
• LPN license in Wisconsin (WI).
• Ability to deliver direct resident care per plan.
• Ability to document care in resident records.
• Administer prescribed medications under policies.

Responsibilities

• Deliver direct resident care per the care plan.
• Record resident care in official records.
• Assist RN in preparing and maintaining care plans.
• Provide information to assist RN in care plan.
• Administer prescribed treatments and medications per policy.
• Notify RN of significant findings or changes in condition.
• Involve patient and/or family in care when appropriate.
