Job summary

Amedisys Home Health is hiring a Licensed Practical Nurse for full-time daytime shifts covering the Greater Harrisburg area and Northern York County. You will support patient care at home, collaborating with RNs and physicians to implement plans of care and ensure timely, accurate documentation.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify them if patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique. Complete documentation timely and accurately.
