Job summary

Amedisys is seeking a licensed practical nurse to join our home health team in North Carolina. The role supports patients in their home with a focus on safe, patient-centered care. A sign-on bonus of 10,000 USD highlights our commitment to attracting experienced LPNs.

Responsibilities include care planning, medication reconciliation, vital signs, and patient education. LPN license and CPR certification are required, along with a valid driver's license.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify when condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. Foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
