Job summary

Open Sky Community Services is seeking a Licensed Practical Nurse for a community-based, patient-centric role. Work in a supportive environment with slower pace than hospital settings, focusing on long-term connections with individuals receiving care.

The LPN collaborates with the RN to meet medical needs, acts as liaison with families and other providers, and helps implement health standards and nutrition plans. A MA LPN license, solid documentation, and strong communication are required.

Qualifications

• Excellent assessment, documentation, and verbal communication skills required.
• Valid driver’s license and acceptable driving history required.
• Experience with adults with disabilities preferred.

Responsibilities

• Oversee medication ordering, counts and refills following physician orders.
• Monitor medical status and transport to medical appointments or delegate transport.
• Advocate for individuals with physicians and other service providers.
• Collaborate with staff to implement health and wellness standards, including nutritious meals.
• Attend ISP meetings and complete required ISP documentation.

Workdays can be a challenge at times, however, with a pace slower than a hospital setting and the ability to work with people on a long‑term basis, Open Sky LPNs are provided a unique opportunity for those who are looking for a clinically based, supportive and nurturing role.

The Licensed Practical Nurse works in close collaboration with the Registered Nurse to ensure the medical needs of people supported are met. This position involves acting as a liaison between the program and other stakeholders, including family members and other medical providers.

Other Key Responsibilities:

• Oversee medication ordering, counts and refills following physicians written orders.

• Monitor individual’s medical status and transport, or delegate transportation to and from medical appointments.

• Advocate for individuals with physicians and other service providers.

• In collaboration with the staff team, ensure health and wellness standards are being implemented including development of nutritious meals.

• Attend all Individualized Support Plan (ISP) meetings and complete necessary documentation for the ISPs.

Qualifications:

• High School Diploma, GED or equivalent, required.

• Certified Licensed Practical Nurse in Massachusetts, required.

• Excellent assessment, documentation, and verbal communication skills, required.

• Valid driver’s license and acceptable driving history, required.

• Experience with adults with disabilities, preferred.

Pay Range:

$31.25-$35.13
