Job summary

Open Sky Community Services seeks a Licensed Practical Nurse to provide clinical, community-based care in a supportive, person-centered setting in Massachusetts. The role focuses on building long-term connections with clients outside traditional office or hospital settings, with a pace slower than hospital work and a commitment to individualized support.

You will oversee medications, monitor health, advocate with physicians, and help implement wellness standards alongside the team, including ISP

Qualifications

• High School Diploma, GED or equivalent, required.
• Certified Licensed Practical Nurse in Massachusetts, required.
• Excellent assessment, documentation, and verbal communication skills, required.
• Valid driver’s license and acceptable driving history, required.
• Experience with adults with disabilities, preferred.

Responsibilities

• Oversee medication ordering, counts and refills following physicians written orders.
• Monitor individual’s medical status and transport, or delegate transportation to and from medical appointments.
• Advocate for individuals with physicians and other service providers.
• In collaboration with the staff team, ensure health and wellness standards are being implemented including development of nutritious meals.
• Attend all Individualized Support Plan (ISP) meetings and complete necessary documentation for the ISPs.

Workdays can be a challenge at times, however, with a pace slower than a hospital setting and the ability to work with people on a long-term basis, Open Sky LPNs are provided a unique opportunity for those who are looking for a clinically based, supportive and nurturing role.
