Job summary

Vivo Healthcare's Gandy Post-Acute and Rehab is seeking an LPN to join our 160-bed skilled nursing facility in Tampa. You will provide direct patient care under the supervision of RNs, administer medications, monitor vitals, and support daily living activities.

You will collaborate with a multidisciplinary team to develop care plans and ensure a compassionate, resident-centered experience. The role offers flexible shifts across 7a-7p and 7p-7a, with opportunities for pay differentials and bonus

Responsibilities

• Provide direct patient care, including administering medications, treatments, and therapies as prescribed by physicians
• Conduct vital signs, monitor patient conditions, and report any changes or concerns to the healthcare team
• Assist patients with activities of daily living (ADLs), such as bathing, dressing, grooming, and toileting
• Maintain accurate and detailed records of patient care, including progress notes and medication administration records
• Collaborate with other members of the healthcare team, including registered nurses, physical therapists, and occupational therapists, to develop and implement comprehensive care plans
• Provide emotional support and comfort to patients and families, promoting a positive and respectful patient experience
