Quality Talent Group represents a trusted network of skilled nursing and rehabilitation centers in New York seeking a Licensed Practical Nurse (LPN) to join a patient-first team in West Hempstead. Options for Full-time, Part-time, and PRN shifts with Day, Evening, and Night availability are offered.

You will deliver direct nursing care, administer medications, document care plans, and educate residents and families.

Qualifications

• Active LPN license in the state of practice.
• Recent graduates are encouraged to apply.
• Skilled in medication administration and patient care procedures.
• Strong documentation, time management, and communication skills.
• Comfortable using EHR and digital charting systems.

Responsibilities

• Deliver direct nursing care to residents in skilled nursing and long-term care settings.

• Implement and evaluate individualized care plans.
• Record vital signs and monitor patient conditions.
• Supervise and guide other nursing personnel when required.
• Educate patients, families, and caregivers as directed.
