Job summary

Merakey is seeking a Licensed Practical Nurse to join our Adult IDD team in Warminster, traveling to group homes across Bucks, Montgomery, and Philadelphia counties to provide direct nursing care.

The role includes administering medications, performing quarterly assessments, educating staff, and coordinating with the nursing supervisor; strong communication and PA LPN license required; competitive hourly pay with shift differentials.

Qualifications

• Graduation from an accredited practical/vocational nursing program.
• Current PA LPN license.
• At least 1 year of LPN experience preferred.
• Valid PA driver's license and reliable transportation.
• Ability to lift up to 50 lbs occasionally, up to 20 lbs frequently, and up to 10 lbs continuously.

Responsibilities

• Visit group homes, deliver medications, update MARs and Treatment Logs, conduct nursing assessments on residents to identify signs of illness, and report findings to the nursing supervisor.
• Identify health issues and follow through to resolution, documenting all actions.
• Review, transcribe, and administer physician's orders accurately.
• Collaborate with healthcare professionals to coordinate care, keeping the nursing supervisor informed.
• Provide health education to residents and their families.
• Conduct in-service training for staff to ensure protocol compliance.
• Respond to emergencies calmly and effectively.
• Perform general nursing duties, including traveling to group homes as a visiting nurse, conducting quarterly physical assessments, and monthly breast exams.

Job description

We are seeking aLicensed Practical Nurseto join our Adult IDD team. Each day will start at our facility in Warminster and then work at our group homes located in Bucks, Montgomery, and Philadelphia counties.

As a Licensed Practical Nurse (Group Home Visiting Nurse) at AVS Merakey, you'll provide direct nursing care to individuals with developmental and physical disabilities. Your responsibilities will include traveling to our group homes, clinical assessments, monitoring acuity, teaching staff on residents' care plan and medical needs, delivering medications to group homes, updating MAR and Treatment logs.

Earn:

$32 / hour + Enhanced Pay Eligible.

NEW Shift Differentials

• Monday – Thursday | 2:30 PM – 7:00 AM + $4.00 per hour
• Friday 2:30 PM – Saturday 7:00 AM + $4.00 per hour
• Saturday 6:30 AM – Monday 7:00 AM + $6.00 per hour

Shift:

M-F noon - 8:00 PM or 12:30 PM - 8:30 PM.

Essential Job Functions:

• Perform general nursing duties, including traveling to group homes as a visiting nurse, conduct quarterly physical assessments, and monthly breast exam.

The ideal candidate will possess the following qualifications:

• Graduation from an accredited practical/vocational nursing program.
• Current license as an LPN in the Commonwealth of PA.
• Preferably, at least one year of experience as an LPN.
• Genuine interest in working with individuals with physical and developmental disabilities.
• Possession of a current and valid PA driver's license, along with reliable transportation.
• Physical capability to comfortably exert up to 50lbs occasionally, and/or up to 20lbs frequently, and/or up to 10lbs continuously.
