Job summary

Traditional Home Health Care seeks an LPN to provide skilled nursing care in patients' homes across multiple PA locations. You will assess patients, follow physicians' orders, and coordinate with the care team to ensure quality outcomes.

Responsibilities include timely documentation, patient education, and referrals to community resources. A PA LPN license and driver's license are required, with willingness to travel to cases as needed.

Qualifications

• Current license as a Practical Nurse (LPN) in Pennsylvania.
• Must have current driver's license.
• Ability to travel to case assignments.
• Excellent assessment skills, documentation skills and ability to meet deadlines.
• Ability to work independently with minimal supervision and manage time effectively.
• Strong interpersonal skills.
• Demonstrates good verbal and written communication, and organizational skills.
• Acute care and/or home health experience is preferred.

• Informs patients of other services available and makes referrals to community resources as needed.

Job Responsibilities Include, But Not Limited To The Following

Requirements
