Job summary

Amedisys is hiring a licensed practical nurse for full-time day shifts to provide in-home patient care. You will verify plans of care with the RN, collaborate with physicians, and support patients in healing at home.

Responsibilities include vital signs, medication reconciliation, patient education, and timely documentation. Requires 1 year LPN experience, active LPN license, CPR, and a valid driver's license.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. Foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
