Job summary

The Christ Hospital Home Health Care is seeking a Licensed Practical Nurse to provide skilled nursing care in patients’ homes within the Kentucky area. Schedule is Monday–Friday, 8:15 AM–5:00 PM with on-call requirements, and travel between assigned locations is expected.

Qualifications

• Licensed Practical Nurse with current license in the state of employment.
• Minimum of one year of experience as an LPN in an acute care setting is required; two years preferred.
• Home care experience is desired.
• Ability to communicate effectively with patients, families, physicians and co-workers.

Responsibilities

• Provides skilled nursing care in a patient’s home as ordered by the attending physician.
• Observes and monitors patient conditions and notifies RN/CM of changes.
• Administers medications as prescribed.
• Performs patient and family education on disease processes and care plans.
• Maintains documentation per Medicare/agency guidelines.
