Reliant Care Management, LLC is looking for a compassionate LPN to provide exceptional care to our residents in Trenton, Missouri. The role involves conducting assessments, managing admissions and discharges, providing wound care, and leading CNAs and CMTs. Candidates must possess a current LPN license and demonstrate empathy, integrity, and a strong work ethic. Job Responsibilities and Duties:

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation
• Provide wound care: follow protocols, irrigate, pack, and remove dressings
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations
